Home First

Home First is a significant shift in health care thinking. When a person enters a hospital with an acute episode, every effort is made to ensure adequate resources are in place to support the person to ultimately go home on discharge and still have the appropriate care. 

Essentially, the Home First program is about providing the right care, in the right place, at the right time and for the right cost. 

The following video further explains how this program is changing care in Ontario.

 

The South East LHIN has the highest proportion in the providence of residents aged 65 years of age or older. By 2016, it is estimated that one in five residents will be 65 years of age or older.

Programs such as Home First, SMILE, and BSO were created to address the needs of our frail elderly residents who account for a high proportion of hospital and emergency departments use. 

Addressing the health and wellness needs of an older patient population, these projects and programs were built on service strengths, acknowledging service gaps and on correcting system integration-type problems such as moving patients out of hospital and into their homes when they no longer need acute care. 

Bringing in the needed services, these seniors can recuperate in the most appropriate setting…home.
