Reporting and Compliance
As a requirement of the Local Health System Integration Act, 2006 (LHSIA), the South East LHIN has a Service Accountability Agreement (SAA) in place with each Health Service Provider (HSP) that it funds. The current 2014-2017 Multi-sector Service Accountability Agreements or M-SAA for the Community Health Centre (CHC), Community Care Access Centre (CCAC), Addictions & Mental Health (AMH) services and Community Support Services (CSS) are in place and can be found on our Accountability Agreements webpage.

Annual Reconciliation Report (ARR)
Below are the resources presented to the Community Health Service Providers required to submit the Excel based reporting tool designed specifically for year-end reporting and reconciled by the Ministry of Health and Long Term Care’s Financial Management Branch (FMB). The ARR provides the ministry with information on an agency’s annual financial performance. At the end of each fiscal year, actual expenditures and services must be reconciled with ministry funding for accountability purposes. The reconciliation is done to enable the ministry to identify funds that have to be recovered.
