Primary Health Care Forum

The South East LHIN's primary Health Care Forum is a networking event geared for primary health care providers. The annual event brings together primary health care professionals working in the South East LHIN region and fosters information, knowledge, and best practices sharing within the primary health care sector and across health sectors.

Shifting Winds and Charting New Courses: Primary Health Care and Health System Transformation

The 2015 Primary Health Care Forum focused on different aspects of Health System Transformation, and how these transformations can arise from Primary Health Care practices and impact patient health, as well as, the entire health care system.

Presentation and workshops were designed to help participants chart new courses in a changing environment. The Forum's learning objectives were to:

  1. Demonstrate and illustrate Primary Health Care’s key role in contributing to effective health system transformation and in providing patient centered care in an integrated system.
  2. Understand and apply the general principles of Health Links, the criteria for Coordinated Care Plans, and other improvement initiatives addressing the social determinants of health to positively impact health care and quality of life.
  3. Discover new approaches and “Bold Ideas” that will positively impact system-wide care and patient outcomes.
The event's purpose was to reveal how primary care driven initiatives can contribute to improving patient health and transforming the health system. Workshops and presentations focused on: 
  • How are Health Links transforming the health care system?
  • How is Information Technology transforming the health care system?
  • How is Quality Improvement transforming the health care system?
  • How is Patient Engagement transforming the health care system?
  • What are some “Bold Ideas” that could transform the health care system?

MAINTENANCE OF CERTIFICATION
Attendance at this program entitles certified Canadian College of Health Leaders members (CHE / Fellow) to 3.25 Category II credits towards their maintenance of certification requirement.

This program has been accredited by the College of Family Physicians of Canada and the Ontario Chapter for up to 6 Mainpro-M1 credits.
