Seeking Membership for the South East Regional Palliative Care Network Steering Committee

We, in the South East, know that comfort, dignity, and quality of life is important to those with life-limiting illness and their families and caregivers. That's why, in partnership with patients, family members, healthcare providers, and other community citizens, we need your help to transform our palliative care approach to improve the individual’s and family’s experience of care and ensure that every person in the South East feels supported in their time of need. Applying a grass-roots approach to enhancing our respective communities; ability to realize this vision, we're looking for local leaders who have the passion and ability to motivate and lead others. You will work with about 12-15 like-minded individuals to realize this vision across the south east.


You share a common vision and a belief that we can do better. You inspire action and have the ability to make decisions in your own organization and/or sphere of influence. You have the belief that we can enhance our collective ability to deliver high-quality, high-value, integrated, and patient-centred palliative care across all sectors and across the entire spectrum of palliative care from presentation/ diagnosis of life-limiting disease to end-of-life care and bereavement. You are a patient, family member, caregiver, health care provider, leader, or a combination thereof. And you know that being accountable means knowing what we're striving for, how we're performing, and being committed to achieving results together;


In alignment with direction from the Ontario Palliative Care Network, the steering committee will provide advice to the South East Local Health Integration Network (LHIN) and the Regional Vice President of the South East Regional Cancer Program on the development of an integrated and coordinated system of palliative care. Members will provide leadership, oversight, and support to organizations within the region involved in the implementation of a strategy for achieving a high performing palliative care system using continuous quality improvement.
