Primary Care Physician

As part of an inter-disciplinary team Belleville and Quinte West Community Health Centre (BQWCHC) offers opportunities for physicians who are seeking a challenging, rewarding position providing comprehensive primary care services to clients at all ages and stages of the life cycle with an emphasis on those who are high risk and/or are experiencing barriers to accessing services. Our health equity focus brings together population health and the social determinants of health within an interdisciplinary primary health care model.

Registered Diabetes Nurse Educator

As a member of an inter-disciplinary team, the Registered Diabetes Nurse Educator provides community-based nursing functions including prevention and management of adult diabetes and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the communities that are affected by diabetes. The Registered Diabetes Nurse Educator will act as an ambassador for the Rideau Valley Diabetes Service/ Rideau Community Health Services in the community and promote positive partnerships with other providers to maximize the capacity of Lanark, Leeds and Grenville to support the prevention and management of diabetes.

Project Lead, Quinte Sub-Region - February 15, 2019 at 4 p.m.

The Project Lead supports the Sub-Region’s goal to improve access to high quality care through the development of sustainable systems of integrated care with the focus on population needs planning, improving health outcomes, experience for patients, and cost to the system as a whole. The Project Lead will provide logistical, project management, and strategic support to the Quinte Sub-Region Integration Table and Working Groups comprised of representatives from organizations that support care to the population.

Nurse Practitioner - February 8, 2019 at 5 p.m.

As part of an inter-disciplinary team BQWCHC offers opportunities for nurse practitioners who are seeking a challenging, rewarding position providing comprehensive primary care services to clients at all ages and stages of the life cycle with an emphasis on those who are high risk and/or are experiencing barriers to accessing services. Nurse Practitioners work as members of an interdisciplinary team that includes physicians, nurses, social workers, pharmacist, dentist, dietitian, and others to provide services to clients of BQWCHC. New Primary Care NP grads will be considered..

Clinical Social Worker/Counsellor - February 15, 2019

As a member of the interdisciplinary Primary Care team, the Social Worker is responsible for providing a wide range of services including: biopsychosocial assessments, individual counselling and facilitating psycho-educational groups for clients. The Social Worker will collaborate and advocate with partners, community agencies and will have a strong understanding of equity as well as experience in working with diverse populations including low-income communities, people living with chronic health and/or mental health issues, addiction struggles, older adults, culturally diverse, LGBTQ+ and others. With a generalist approach, the Social Worker will show competency and comfort in using various modalities of therapy.

Pharmacist - February 15, 2019

Practices patient-centred care to achieve optimal drug therapy outcomes for enrolled patients, as part of a team of primary care health professionals. The Pharmacist will provide pharmaceutical care to patients by optimizing medication therapy and acting as a resource to the team. The Pharmacist will provide regular educational updates with respect to drug-related guidelines and evidence-based research to patients, physicians and other professional staff within the clinic.
